Do You Need a Facelift in New York or TWT?
Facial cosmetic surgery variations are almost infinite, as they should be. Patients deserve to have a surgical plan tuned to their exact needs. Men and women whose faces show just the early signs of aging may elect eyelid surgery, or perhaps a brow lift/eyelid lift combination. Those who are looking for just a little tune up might be candidates for Botox and fillers.
On the other end of the spectrum are patients who choose “the works” in facial rejuvenation, or TWT – “the whole thing” as we call our extended New York facelift. That means a browlift, eyelid surgery, facelift and neck lift. And yes, TWT can be performed all in one operation.
Here’s how we perform the ultimate procedure for facial rejuvenation.
The Brow Lift is First
Because each technique that elevates tissues has some effect on structures beneath, we start with the brow lift. Elevating the forehead will improve brow position and smooth horizontal wrinkles. This procedure helps rejuvenate the upper face and open up the eyes without giving a look of permanent surprise.
Then, Upper and Lower Eyelid Surgery
For New York eyelid surgery patients, we first start with incisions in the crease of the upper lid that will be invisible upon healing. We remove just enough skin to return a youthful look to the eyes by reversing droopiness. If needed, we can weaken muscles that cause worry lines between the eyebrows too.
Lower lid incisions are also hidden by placing them just below the lash line. As needed, we can reduce or reposition fat pads and pull up lax skin. If a patient would benefit from cheek implants, we can insert them through these same incisions.
Eyelid surgery is one of our favorite procedures due to its flexibility and profound impact on patients’ appearance. Read more about what we can accomplish here.
The Facelift is Next
Next comes the facelift for our New York TWT patients. We make incisions around the back of the ears and in the hairline so we can gently elevate both the skin and the underlying muscle structures for long lasting results. We lift toward the temples to avoid an unnatural catlike or windswept look. Patients who elect a full facelift in New York with us can expect smoothing of the cheeks and a less jowly appearance of the jawline.

Finally, the Neck Lift
With jowls and cheeks addressed, it’s time to freshen up the neck. Neck skin often loses elasticity as people age, and sagginess can reveal vertical muscle bands or a fatty double chin. For some people, genetic predisposition to a problematic neck can lead to an aged appearance far too soon and a neck lift might be an appropriate standalone procedure. For others, it just makes sense to tighten loose tissues as the finishing touch to TWT. It’s a relatively straightforward operation, usually requiring a small chin incision and light liposuction.
Pros and Cons of TWT
We’re not going to lie—this extended facelift procedure takes a few hours in the operating room. And it costs more than any one of these rejuvenating surgeries by itself. But the benefits are transformative for the right patient.
Some patients—typically people in or beyond middle age, with average or less-than-great skin resilience—will truly benefit from top to bottom rejuvenation. This is even more the case when environmental damage from sun, a dry climate, wind and pollutants have had their impact. Choosing TWT means surgery is over with all at once, with just one recovery period. And, patients save some money by having just one round of anesthesia.
You may or may not know whether TWT is a good call for you, or whether just one or two of the other choices would suffice. That’s where we come in! Call us at 212-570-6980 to arrange a consultation, and we’ll give you our recommendation and a look at all your options.
